Lloyd HVAC Services Inc. Commercial Preventative Maintenance — Terms & Conditions 1. Plan & Eligibility The Commercial Preventative Maintenance Plan provides scheduled preventative maintenance for commercial HVAC equipment, starting at $300 per appliance annually. Final pricing is confirmed based on the equipment, quantity and required maintenance schedule. Coverage applies only to equipment specifically enrolled in the plan and located at the registered service address. 2. Preventative Maintenance The plan includes scheduled preventative maintenance in accordance with the agreed maintenance frequency (Annual, Semi-Annual, or Quarterly). Maintenance includes applicable inspection, cleaning, safety and operational checks, and a maintenance report with findings and recommendations. Maintenance does not guarantee that equipment will remain free from breakdowns or require no future repairs. 3. Initial Equipment Condition Equipment must be operational and in serviceable condition when enrolled. Lloyd HVAC Services Inc. may require an initial inspection or maintenance visit to establish the condition of the equipment. Pre-existing conditions, previously identified deficiencies, and existing equipment failures are not eligible for plan repair coverage. 4. Scheduling & Customer Access Customers are responsible for providing safe and reasonable access to enrolled equipment at the scheduled appointment time, including any required building access, parking, or rooftop access arrangements. Missed appointments, same-day cancellations or appointments where equipment cannot be safely accessed may be subject to applicable service or cancellation fees. 5. Invoicing & Payments Annual commercial plan fees are invoiced per the terms confirmed at enrollment. Plan benefits may be suspended if invoices become past due. 6. Cancellation If the customer cancels after receiving preventative maintenance or other plan benefits, any outstanding balance associated with services or benefits already received may become immediately due. Plan benefits end upon cancellation or termination of the plan. 7. Equipment Replacement If enrolled equipment is replaced, coverage on the original equipment ends. Where replacement equipment is installed by Lloyd HVAC Services Inc., the customer may request that the remaining plan coverage be transferred to the new equipment, subject to eligibility. 8. Service Recommendations Any deficiencies identified during maintenance will be documented. Repairs will be quoted separately and require customer approval before proceeding. Customers remain responsible for addressing recommended safety-related or operational deficiencies. 9. Limitations The Commercial Preventative Maintenance Plan is a preventative maintenance program. It is not an insurance policy, extended manufacturer warranty or guarantee against equipment failure. Lloyd HVAC Services Inc. is not responsible for failures or damage resulting from circumstances outside the reasonable scope of HVAC preventative maintenance. 10. Acceptance of Terms Enrollment and continued participation in the Commercial Preventative Maintenance Plan confirms the customer's acceptance of these Terms & Conditions.
